Location: Outagamie County, WI
Schedule: Full‑time, Monday‑Friday, first shift (8 am‑4 pm or 9 am‑5 pm)
Flexibility: Required to meet family needs

Are you passionate about making a difference in the lives of children and families? Lutheran Social Services is seeking a Care Coordinator to support families through the Children’s Long‑Term Support (CLTS) Waiver Program
—a federally funded Medicaid program serving children with developmental, physical, or severe emotional disabilities.
As a CLTS Care Coordinator
, you will be the main point of contact for families, helping them access services that keep children safe and thriving in their homes and communities.
- Assess functional abilities and needs using required tools
- Develop and implement individualized service plans with families and providers
- Coordinate and authorize services per plan
- Facilitate family‑centered team meetings
- Maintain accurate documentation and client records
- Apply trauma‑informed, evidence‑based practices
- Participate in staff development and clinical supervision

- Education: Bachelor’s degree in Social Work or a related human services field
- Experience: Minimum 1 year working with children/youth with disabilities
- Requirements:
- Valid driver's license and reliable transportation
- Ability to pass background and caregiver checks
- Strong documentation and communication skills
- Computer proficiency, including electronic health records
- Daily travel within the community may be required.
