Family Preservation Social Worker

Location: Hybrid - Wellspring Gaylord

Reports to: Family Services FTBS Supervisor - Gaylord

Position Type: Full-Time | Salary - $43,000/year (based on experience)

Wellspring is a statewide nonprofit dedicated to serving seniors, families, and children throughout Michigan. We are currently seeking a Family Preservation Social Worker to join our Family Together Building Solutions (FTBS) Program, supporting families throughout Northern Michigan, including Otsego, Crawford, Iosco, Alcona, Ogemaw, Roscommon, and Oscoda counties. You will provide home-based, family-centered services designed to help families stay together, strengthen relationships, and build skills for long-term success.

• Adhere to the FTBS model of service (strength-based, non-deficit approach).
• Provide families with concrete services, including assistance with housing, transportation, home cleaning, maintenance, and repair.
• Deliver skills training in areas such as parenting, communication, self-advocacy, and self-sufficiency.
• Offer short-term counseling and crisis intervention for families in need.
• Coordinate with external providers to arrange counseling, medical care, educational support, housing resources, and utility assistance.
• Prepare timely documentation, including service plans, progress reports, termination summaries, goal sheets, and weekly reports.
• Present cases for termination conferences and recommend follow-up services.
• Maintain 24-hour availability for clients and provide a flexible work schedule based on family needs.
• Conduct face-to-face contact with families at the frequency required by contract and family needs.
• Maintain weekly contact with referring workers and complete follow-up evaluations at 3, 6, and 12 months post-service.
• Transport clients in your own vehicle as needed.

• Bachelor’s degree in social work or a human services–related field required.
• Experience working with children and families preferred.
• Reliable, compassionate, and committed to providing high-quality service.
• Strong organizational, communication, and problem-solving skills.
• Current and valid driver’s license, vehicle registration and auto insurance required.

• Hybrid-Remote position – home-based with daily travel throughout Otsego, Crawford, Iosco, Alcona, Ogemaw, Roscommon, and Oscoda counties.
• Flexible schedule with 24-hour call availability as needed.
• Team meetings and supervision held 1–2 times per week.
• Salary: $43,000/year (based on experience).
