Overview

In this full time role, the Case Manager will provide field/home and/or office-based services to clients including assessments, treatment planning, individual & group and family therapy, discharge planning, and participation in Child and Family Team meetings.

While this role is based out of our Glendale office, it is a field-based role serving clients throughout LA’s Service Planning Area
2. This area includes, but is not limited to:
Calabasas, Glendale, San Fernando, Santa Clarita, Agoura Hills, Castaic, Chatsworth, Northridge, Pacoima, Reseda, Saugus, Sherman Oaks, Sunland, Tujunga, and Van Nuys.

  • Manage and supervise a caseload of 23 families/clients, and facilitating monthly, face-to-face home visits scheduling to best meet the need of the client and family.
  • When needed, respond to and attend to details of crisis situations that may arise during working (and non-working hours, if appropriate).
  • Conduct intake assessments to obtain information necessary to determine which services are appropriate and available to address the family’s needs.
  • Develop, in partnership with the family, a written Individualized Service Plan (ISP).
  • Supervise the implementation, progress, and completion of the ISP.
  • Ensure linkage referral is complete and able to be accessed by client and provide other community resources to promote family health and stabilization.
  • Complete required reports that communicate quality treatment services, such as quarterly reports, special incident reports, relationships, treatment team meetings, group, individual, and family sessions, case management activity. (i.e., Mental Health Documentation), and completion/termination summary reports and linkage forms.
  • Complete monthly progress notes and activity logs and submit to Director on a timely basis.
  • Complete PAPF at intake, annually and at discharge; complete CAFAS as pre-determined intervals.
  • Cultivate contacts and relationships with outside agencies and individuals that have both direct and indirect bearing on the case. These include:
    • County Social Workers
    • County Eligibility Technicians
    • School Personnel
    • Community Mental Health Agencies
    • Regional Center Personnel
  • Provide intensive home-based mental health services, if deemed appropriate by treatment team.
  • Attend, as needed, client-focused meetings such as IEP meetings, Wrap Around meetings, CFTs, target behavioral specialist meetings and psychiatry appointments.
  • Assist with coordinating events or special services and supports, as needed.
  • Attend/facilitate/co-lead support groups, as requested.
  • Complete all training hours as required by the contract.
  • Attend treatment team meetings, in addition to, individual and group supervisions.
Required Qualifications
  • Bi-lingual (English and Spanish)
  • A bachelor’s degree from an accredited college or university in one of the following: social work; marriage, family and child counseling; child psychology; child development; counseling psychology; social psychology; clinical psychology; educational psychology; or education with emphasis on counseling.
  • Experience working within child welfare and family systems, and/or trauma-informed work.
  • Adoption-related experience is preferred.
  • Must obtain and maintain record clearance free from criminal convictions.
  • Must have and maintain a valid California Driver License and clean driving record in accordance with Olive Crest’s auto insurance policy.
