Prairieland Service Coordination, Inc. is seeking Case Managers / QIDP’s who reside in the following counties:
Champaign, Ford, Iroquois and Vermilion. A bachelor’s degree in a human services‑related field, a one‑year cumulative experience providing direct service to individuals with developmental disabilities (which can be earned through employment, volunteer or family care), and QIDP eligibility are required. The role offers excellent benefits, a regular schedule of 8:00‑am to 4:30‑pm Monday through Friday, and flexibility to work partially from home.

General

Description

Under the direct supervision of the Program Manager, the Case Manager assesses individual needs and ensures the provision of necessary services through communication, service coordination, linkage with provider agencies, service monitoring and individual advocacy.

Job Responsibilities
  • Provide services in the counties served by Prairieland Service Coordination, Inc. (23 counties); may need to work in a county outside usual caseload in case of staff illness or departure.
  • Complete the PAS process following the PAS manual procedures.
  • Advocate on behalf of the individual and/or family to obtain eligible services, ensuring direct involvement of the individual and family.
  • Review medical, residential, vocational, educational, legal and financial status through Visiting Notes as required.
  • Schedule and complete the Discovery and Personal Plan process annually or as needed.
  • Maintain individual files in accordance with program and agency guidelines.
  • Provide problem‑solving consultation with provider agencies per the DHS Conflict Resolution guideline.
  • Coordinate specialized services for eligible Bogard individuals and submit summary reports to external agencies as appropriate.
  • Monitor services provided to Bogard class members monthly and advocate for unmet needs.
  • Enter billings daily into Birdseye to ensure timely payroll reporting.
  • Participate in scheduled staff meetings and mandatory training sessions as recommended by the Executive Director or Program Manager.
  • Alert the Program Manager of PSCI to problematic issues, trends or service gaps.
  • Perform additional related work as required or requested by the Executive Director or Program Manager.
Supervisory Responsibilities

None.

Job Qualifications
  • Bachelor's degree in human services and QIDP eligibility.
  • Strong organizational skills with the ability to plan and follow a schedule on daily, weekly, monthly and annual bases.
  • Working knowledge of the developmental disabilities service delivery system, including eligibility requirements and procedures.
  • Effective interpersonal skills to work with professionals, paraprofessionals, social service organizations and clients (plus families).
  • Ability to work well in teams and independently from a home office with self‑discipline and motivation.
  • Flexibility to work hours outside the normal workweek.
  • Willingness to follow directions, accept constructive criticism and take responsibility for actions.
  • Adherence to the Employee Code of Ethics with professional diplomatic oral and written communication.
  • Reliable transportation and valid Illinois Driver’s License; acceptable auto insurance and vehicle record.
Physical Expectations
  • Ability to read, write and communicate effectively.
  • Ability to ambulate, sit and bend.
  • Ability to carry files.
  • Ability to lift up to 5 lbs. or pull a carrier.
