Location: Sullivan

We are looking to add to our Prairieland family in Decatur, IL. Prairieland is seeking a Case Manager/QIDP to serve Moultrie, Douglas, and Coles counties. A bachelor’s degree in human services and one‑year cumulative experience providing direct service to individuals with developmental disabilities is required. The one‑year cumulative experience requirement can be met through employment, volunteer or family care. Excellent benefits.

Schedule 8:00 am to 4:30 pm, Monday through Friday, with flexibility to work partially from home.

General Description

Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager assesses individual needs and ensures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring, and individual advocacy.

Job Responsibilities
  • Prairieland Service Coordination Inc. provides services in twenty‑three counties and at any given time a Case Manager may be requested to work in a county typically outside their caseload's geographic area (i.e., staff illness, staff resignation/termination).
  • Completes the PAS process, following the PAS manual procedures.
  • Advocates on behalf of the individual and/or family to assist the individual in obtaining services to which he/she is eligible and has chosen, ensuring the direct involvement and participation of the individual and their family throughout the process.
  • Reviews individual’s medical, residential, vocational, educational, legal and financial status needs and individual choices through Visiting Notes, as required.
  • Schedules and completes Discovery and Personal Plan processes annually, or as required.
  • Maintains individual files as specified by program and agency guidelines.
  • Provides problem‑solving consultation with provider agencies following the DHS Conflict Resolution guideline.
  • Schedules and completes all Visits, Discovery, and Personal Plan processes as required.
  • Coordinates specialized services for eligible Bogard individuals as needed and submits summary reports to external agencies as appropriate.
  • Monitors services provided to Bogard class members on a monthly basis and advocates for any unmet needs.
  • Enters billings daily into Birdseye to ensure all time reporting for payroll is submitted on time and accurately.
  • Participates in scheduled all staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
  • Alerts the Program Manager of PSCI to problematic issues, trends or service gaps.
  • Performs other related work as required or requested by the Executive Director and/or Program Manager.

Job Qualifications
  • A bachelor’s degree in human services is required and QIDP eligibility as established by the Department of Human Services.
  • Has the ability to organize, plan and follow a schedule on a daily, weekly, monthly and annual basis.
  • Demonstrates a working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
  • Demonstrates ability to work and interact effectively with professionals, paraprofessionals, social service organizations and clients including their families.
  • Can work well in teams of co‑workers and in a home office environment using self‑discipline and self‑motivation skills with the ability to work without direct supervision.
  • Is flexible to work hours outside the normal workweek.
  • Is willing to follow directions, accept constructive criticism and accept responsibility for their own actions.
  • Follows the Employee Code of Ethics Guideline by presenting oneself in a professional manner through diplomatic oral and written communication.
  • Has reliable transportation to and from work or can access transportation to work and locations where individuals live/work that may be assigned to your caseload; maintains a valid Illinois Driver’s License, personal auto insurance and a good motor vehicle record.

Hourly Wage Range: $19.95 - $21.63. Salary: $19.95 - $21.63 per hour.
