Position Title

Level2–3 Qualified Social Workers – Long Term Conditions Teams (Social Worker posts in our other teams are advertised separately).

Location & Hours

Full time, 37 hours per week – Permanent.

Location:

Bournemouth, Christchurch and Poole.

Salary

Salary ranges from £44,075 (Level2) to £48,226 (Level3) depending on level of experience.

Key Responsibilities
  • Support adults aged 18+ living with long‑term conditions to live safely and independently in the community.
  • Work alongside individuals, carers and families to understand what matters most and identify needed support.
  • Provide rights‑based practice, court work, safeguarding and other services including links to national and local resources.
  • Document and review service plans, risk assessments and statutory reports.
Qualifications & Experience
  • Social work qualification (DipSW, BSc/BASW, MSW, SWDA, or equivalent) and registration.
  • Experience working with adults with long‑term conditions or in a similar setting preferred.
Flexibility & Working Arrangements

Full‑time, part‑time and flexible (hybrid) options are available. Home working and flexible office spaces.
