Position: Housing First Practitioner
We're seeking a proactive Housing First Practitioner in the North Liverpool area to join a specialist homelessness support service on a MAT cover contract until at least January 2027, possibly beyond. This is a 35 hour per week role, participating in a rolling rota working between the hours of 8am and 8pm, required to support 1 in every 6 weekends.

Your new role You'll be responsible for providing intensive, wrap-around support to highly vulnerable individuals who are facing or experiencing homelessness, helping them to sustain or secure tenancies, access community services, and work towards personal goals. The role involves a mix of outreach, home visits, office-based and home working.
Responsibilities:

Deliver high-quality, person-centred, trauma-informed support to individuals facing homelessness, managing your own caseload of up to 8 clients ensuring they receive responsive support tailored to their circumstances
Conduct risk assessments and develop safety plans including crisis planning, de-escalation and harm reduction strategies
Devise personalised support plans to be reviewed regularly, monitoring progress and documenting achievements
Provide outreach support including home visits and
community based work
Work in partnership with multiple agencies and services, including health, social care, probation, housing providers, community groups and other professionals to maintain holistic support
Provide practical, tenancy-related support including housing applications, Property Pool Plus support and benefits advice

What you'll need to succeed

Strong background in supporting vulnerable adults facing homelessness, rough sleeping or multiple complex needs
Strong understanding of trauma-informed and strengths-based practice
Knowledge of homelessness law, mental health, substance use, welfare benefits and housing pathways

Experience of completing risk assessments, crisis planning, safeguarding and devising personalised support plans
Confident lone worker with the ability to manage challenging behaviour with empathy and professional boundaries.
