What you will do
  • Strength‑Based Assessments: Conduct assessments using a strengths‑based approach to promote independence and wellbeing.
  • Integrated Working: Collaborate with partner agencies to deliver coordinated, holistic care.
  • Mental Capacity & Safeguarding: Complete Mental Capacity Assessments and Section 42 enquiries in line with the Care Act 2014.
  • Hospital Discharge: Support safe and timely hospital discharges where required.
  • Duty Work: Manage duty responsibilities and respond to urgent cases effectively.
  • Case Recording: Maintain accurate, timely records and documentation in line with statutory standards.
Qualifications / Requirements Essential
  • Qualified Social Worker with current Social Work England (SWE) registration.
  • Strong working knowledge of the Care Act 2014 and adult safeguarding frameworks.
  • Experience completing Mental Capacity Assessments and Section 42 enquiries.
