We are seeking an experienced Triage Worker to support individuals who are homeless or at risk of homelessness.

Key Responsibilities:

Conduct triage and eligibility assessments Act as the first point of contact for service users Assess needs and identify suitable support pathways Gather and verify supporting documents and information Manage referrals and maintain accurate case records Liaise with local authorities, housing providers, and partner agencies Signpost clients to relevant housing, welfare, and support services Support safeguarding and tenancy sustainment outcomes Requirements:
