Position: Behaviour Change Support Practitioner

Crawley (Hybrid – office-based and remote working available)

Hours:

16-24 per week (day shifts) Duration: 2–3 months Rate: £16.50 per hour (including holiday) Travel:
Required across other sites when needed About the Role We are urgently recruiting a Behaviour Support Change Practitioner to join our established services. This role is vital in supporting individuals with complex needs to achieve positive behavioural and life changes within supported accommodation and community settings. You will work within a supportive team to deliver structured, person-centred interventions that promote independence, emotional wellbeing, and sustainable behaviour change.

The role involves direct support, behaviour management, and close partnership working with external agencies.

Key Responsibilities
• Deliver person-centred, strengths-based support to individuals with complex behavioural needs.
• Conduct needs, behavioural and risk assessments, developing tailored behaviour support and change plans.
• Provide structured keywork sessions, interventions and ongoing support focused on behaviour change and positive outcomes.
• Support individuals to access appropriate services, including housing, health, employment and community resources.
• Maintain accurate case notes, reports and risk/safeguarding documentation.
• Work collaboratively with local authorities, health professionals, social care and other partner agencies.
• Support individuals in developing daily living skills and improving independence.
• Manage challenging behaviours calmly and effectively using de-escalation and positive behaviour support techniques.
• Uphold health & safety, safeguarding and confidentiality requirements at all times. What We’re Looking For Essential:
• Experience supporting individuals with complex needs, including behaviours that challenge.
• Ability to work in a person-centred, trauma-informed and strengths-based way.
• Good understanding of safeguarding, risk management and GDPR.
• Strong communication, interpersonal and report writing skills.
• Ability to prioritise workload, make decisions and remain calm in challenging situations.
• Understanding of support systems such as housing, health or social care services.
• First Aid trained or willing to complete training. Desirable:
• Relevant qualification (e.g. Psychology, Social Work, Health & Social Care).
• Experience working within supported accommodation or community-based services.
• Knowledge of mental health, substance misuse or dual diagnosis.
• Experience delivering structured interventions or behaviour programmes.
