About the Post

An exciting opportunity has come available for a temporary 6 month contract (cover maternity) for an assistance social care practitioner.

We are looking to recruit an enthusiastic and committed individual to join our established team of Social Workers and Assistant Social Care Practitioners. The role requires a confident individual who can assist in the completion of service user and carer assessments of care needs and undertake reviews. You will be required to be creative in coordinating care planning, commissioning, and provisioning services to meet desired outcomes in line with promotion of early intervention and strengths-based social work.

You will be supported to work autonomously and carry an individual caseload, but equally work as part of a team to meet service demands. You will need to have some experience of working in health and social care settings and demonstrate skills in work prioritisation, identifying risks, organising your own workload, and working in accordance with all statutory frameworks and relevant social care legislation, council policies, and procedures.

Based within Westgate Plaza, supporting a busy Adult Social Care Locality Team.
