Starting salary for this role is £32,061/£34,434 DOE (pro rata for part‑time colleagues).

Community Based Adult Social Care Practitioner – Intake

Our team works within the Adult Social Care Hub to support residents over 18 and their carers. We focus on enabling people to minimise dependence and maximise independence through strength‑based, self‑directed support.

What You Can Expect To Be Doing
  • Conduct comprehensive assessments of adults and carers to determine needs and promote social welfare and inclusion.
  • Provide direct support in homes, care homes and hospital settings.
  • Collaborate closely with health partners and other providers to deliver coordinated care.
  • Manage your own workload, ensuring timely completion of visits and documentation.
What We’re Looking For
  • Recent experience working in Adult Social Care or a similar agency with direct public contact.
  • Good understanding of the social and legal context of social care services.
  • Excellent communication skills and the ability to advocate, signpost and give advice to adults.
  • Quick learner who can adapt to a fast‑paced environment and follow established procedures efficiently.

Working as part of a coordinated team, you will benefit from regular supervision, peer support and opportunities to gain relevant qualifications through structured learning and mentoring.

Additional Information
  • Full‑time (37 hours per week) role based in the Bournemouth area.
  • Requires right to work in the UK; role is not eligible for visa sponsorship.
