This role offers a hybrid working model, allowing you to work from home for part of the week while continuing to deliver high-quality assessments in the community. In this role, you will undertake combined assessments of adults with care and support needs alongside assessments of their unpaid carers, ensuring a holistic understanding of each individual’s situation. Working in line with the Social Services and Well‑being (Wales) Act 2014, you will identify needs, strengths, and personal outcomes for both the person and their carer. You will promote wellbeing, independence, and sustainable caring arrangements through the provision of information, advice, assistance, and appropriate referrals to support services.
