Job Description 3 Month Contract With A Local AuthorityJob Purpose
To provide high-quality outreach and home-based Early Help support to children, young people, and families using a whole-family approach. The role focuses on delivering targeted interventions that improve outcomes, reduce risk, prevent escalation to statutory services, and promote the wellbeing and safeguarding of children and young people.
Key Responsibilities- Manage a caseload of children and families requiring targeted Early Help support.
- Complete Early Help Assessments (EHAT) and develop effective support plans.
- Act as Lead Professional, coordinating Team Around the Family (TAF) meetings and multi-agency interventions.
- Provide intensive outreach and home-based support to children, young people, and families with complex needs.
- Support families in developing positive parenting strategies and improving family relationships.
- Assess risk, monitor progress, and review support plans to achieve positive outcomes.
- Work collaboratively with social care, education, health, and partner agencies to deliver coordinated support.
- Maintain accurate, timely, and confidential case records using electronic case management systems.
- Undertake home visits and deliver practical interventions in community settings.
- Support supervised family contact where required.
- Promote safeguarding, equality, diversity, and anti-discriminatory practice in all aspects of the role.
- Attend supervision, training, case reviews, and multi-agency meetings to support continuous professional development and service improvement.
- Experience working with children, young people, and families in a social care, family support, or Early Help environment.
- Knowledge of safeguarding legislation, child protection procedures, and Early Help processes.
- Experience completing assessments and coordinating multi-agency support plans.
- Strong communication, relationship-building, and problem-solving skills.
- Ability to manage a varied caseload and work independently within agreed timescales.
- Competent IT skills with experience maintaining accurate electronic records.
- Enhanced DBS clearance.
- Full UK driving licence and access to a vehicle (desirable, depending on service requirements).
