IMPACTAgewell® is an asset-based approach to integrated care for Older People in Mid and East Antrim, Northern Ireland. The organization focuses on improving the quality of life for older people by addressing their social needs on par with their medical needs and connecting them to their community.

This is a full
-time hybrid role for an IMPACTAgewell Officer. The role involves tasks such as coordinating integrated care services for older people, connecting them to community resources, and advocating for their social well‑being. While the role is based in Broughshane, there is flexibility for remote work.

• Experience in social care, healthcare, or community outreach
• Strong communication and interpersonal skills
• Ability to coordinate and collaborate with multidisciplinary teams
• Knowledge of asset-based approaches to care for older people
• Proficiency in data collection and reporting
• Empathy and understanding of older people's needs
• Ability to work independently and in a team
• Understanding of integrated care model
