Key Responsibilities:

• Assess participant rehabilitation and capacity for improvement

• Ensure alignment of plans with program mandates

• Formulate Rehabilitation Plans with participants and professionals

• Collaborate with multidisciplinary service integrators

• Address barriers and develop proactive strategies

Requirements:

• Bachelor's degree in relevant field and professional certification

• At least three years of rehabilitation service experience

• Experience working with individuals facing health challenges

• Federal security clearance required

• Travel across the region is expected
