Change Grow Live is a charity dedicated to helping service users by providing support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

Full time, 37.5 hours per week (including weekend and evening work). Permanent contract.

The Foreign Nationals Outreach Worker will manage a caseload of foreign nationals experiencing rough sleeping and substance misuse, providing assessments, recovery planning, advocacy and coordination with partner services to support recovery and reintegration.

• Engage with foreign nationals through street‑based outreach sessions at various times of the day and night.
• Manage a small caseload of service users, acting as the main coordinator and providing comprehensive assessments.
• Screen, assess and develop recovery plans with onward referrals.
• Reduce drug and alcohol related harm to service users and the wider community.
• Provide advocacy for access to partnership services.
• Complete planned outreach projects with partner agencies, including street counts and satellite delivery for vulnerable groups.
• Work with service users to support social (re) integration, enabling them to lead meaningful and purposeful lives through recovery, resilience and peer support.
• Collaborate with the Police, the Home Office and anti‑trafficking agencies when necessary to address exploitation and trafficking issues.

• Strong understanding of substance misuse issues and experience working within a similar field.
• Ability to communicate effectively with service users from at least two EU member states (Czech, Slovakian, Lithuanian, Polish, Latvian, Hungarian, Romanian).
• Proactive, able to work on own initiative with excellent time management and prioritising skills.
• Knowledge of services available in various countries of origin.
• Supportive team player with strong interpersonal skills and ability to work in partnership with a wide range of professionals, agencies and stakeholders.
• Understanding of information governance processes and commitment to safeguarding.
• Experience working with people who have been trafficked or are victims of modern‑day slavery.
