Dorset is recruiting a Home First Team Social Worker (Adults) in Dorchester. Hybrid/Remote working with hospital discharge roles across Dorset, supporting patients to return home safely. You will lead complex assessments, coordinate services and work with NHS colleagues to ensure timely discharges.
Applicants must be a qualified social worker, registered with Social Work England, and have the right to work in the UK. Enhanced DBS clearance will be obtained by us.
