What you will do

• Care Act Assessments: Conduct assessments in line with the Care Act 2014 to identify needs and develop tailored care plans.
• Aftercare Responsibilities: Undertake Section 117 Aftercare duties to support recovery and long-term wellbeing.
• Safeguarding: Carry out Adult Safeguarding work to protect vulnerable individuals.
• Strengths-Based Practice: Promote independence through strengths-based approaches and personalised interventions.
• Multidisciplinary Working: Participate in multi-agency meetings with health and social care professionals.
• Service Delivery: Provide high-quality, person-centred support to adults with mental health needs.

Qualifications / Requirements

• Qualified Social Worker with Social Work England registration.
• Experience with in Adult Social Care, ideally within a CMHT setting.
• Strong knowledge of the Care Act 2014 and safeguarding procedures.
• Experience supporting adults with mental health needs.
• Ability to work effectively within multidisciplinary teams.
• Excellent communication, assessment, and organisational skills.
