What you will do

  • Assessment: Conduct comprehensive assessments of adults’ needs, strengths, and support requirements.
  • Care Planning: Develop personalised care and support plans that promote recovery, independence, and wellbeing.
  • Direct Support: Build strong professional relationships through face-to-face engagement and ongoing intervention.
  • Case Management: Manage a varied caseload and ensure timely reviews of support arrangements and outcomes.
  • Multi-Agency Working: Work collaboratively with healthcare professionals, community organisations, and partner agencies to deliver coordinated care.
  • Advocacy: Empower and advocate for individuals, promoting choice, independence, and informed decision-making.
  • Review and Monitoring: Regularly evaluate care plans and adapt interventions to meet changing needs and circumstances.
  • Record Keeping: Maintain accurate, timely, and compliant case records in line with statutory and organisational requirements.
  • Safeguarding: Identify and respond appropriately to safeguarding concerns, ensuring service users receive the protection and support they require.

Qualifications / Requirements

  • Social Work

    Qualification:

    Degree in Social Work or equivalent recognised qualification.
  • Registration: Current Social Work England registration.
  • Experience: Previous experience working within Adult Social Care and Mental Health Services.
  • Mental Health Knowledge: Strong understanding of mental health legislation, safeguarding frameworks, and person-centred practice.
  • Assessment

    Skills:

    Experience undertaking comprehensive assessments, care planning, and risk management.
  • Communication

    Skills:

    Excellent verbal, written, and interpersonal communication abilities.
  • Partnership Working: Ability to work effectively with multidisciplinary teams and external agencies.
  • Organisational

    Skills:

    Strong time management and case management skills within a busy service environment.
