The Role

As a Hospital Discharge Social Worker, you'll be based within the hospital, working as part of a busy multidisciplinary team to support individuals through the Discharge to Assess (D2A) pathway and ensure safe returns home. Working across a 7-day service (with a requirement to work at least one Saturday or Sunday as part of the rota), you'll complete strengths-based assessments, coordinate care, and work alongside health professionals to prevent unnecessary delays and achieve positive outcomes for service users.

Your Responsibilities
  • Completing Care Act Assessments.
  • Supporting individuals through the Discharge to Assess pathway.
  • Arranging and reviewing care packages to facilitate safe discharge home.
  • Managing safeguarding concerns where appropriate.
  • Working closely with NHS colleagues, therapists, ward staff, and community services.
  • Supporting discharge planning from the Emergency Department and hospital wards.
  • Maintaining accurate case records and delivering person-centred interventions.
About You

To be considered, you'll need:

  • A recognised Social Work qualification (BA, MA, DipSW or equivalent).
  • Current Social Work England registration.
  • At least 2-3 years post-qualified experience within Adult Social Care.
  • Previous experience in Hospital Discharge or Discharge to Assess services.
  • A good understanding of the Care Act, safeguarding, and strengths-based practice.
