Social Worker or Advanced Social Worker - Yate

As a Social Worker or Advanced Social Worker, you will join our Promoting Independence and Reablement Team, taking an independence-led approach to ensuring the social, health and welfare needs of adults in South Gloucestershire are met. You will work with individuals who require social care services, alongside their families and carers, to promote independence and deliver clear outcomes that are monitored and regularly reviewed.

You will support individuals referred from the community and through the Sirona Pathway 1 hospital discharge pathway.

You will ensure individuals receive timely and appropriate support, enabling them to live as independently as possible. With a strong emphasis on effective and lawful practice, you will help ensure care and support remain appropriate and deliver best value, while promoting confident professional judgement and defensible decision-making.

• It will be your responsibility to deliver person-centred, strengths-based support, focused on what matters most to the individuals we work with. Using creativity and professional judgement, to achieve the best possible outcomes.
• Using your expertise, you will implement, monitor and review care plans, ensuring actions and outcomes remain relevant and effective. Where needs change, you will adapt plans accordingly to provide appropriate support.
• Working in line with current legislation, policy and best practice, you will promote the wellbeing, dignity and independence of service users. Drawing on your experience, you will make informed recommendations, including the use of reablement where appropriate.
• You will maintain accurate, up-to-date, and confidential records in line with professional standards and council requirements.

Advanced Social Worker Additional Responsibilities:

• Drawing on research and evidence-based practice, you will lead on more complex and high-risk cases, applying advanced professional judgement and taking responsibility for key decisions.
• Acting as a role model, you will support and develop best practice across the team through advice, mentoring and shared learning.
• You will contribute to workforce development, including supporting and supervising students on placement.
• In line with advanced practitioner expectations, you will maintain up-to-date core training and actively engage in continuing professional development.

• It is essential that you hold a degree in Social Work and be registered with Social Work England.
• We are looking for someone who has experience working within a social care setting.
• Given the elements of this role, you will have a strong understanding of asset and strengths-based approaches to adult social care.
• You will be a confident decision-maker with strong problem-solving skills and a genuine passion for working collaboratively with people to achieve positive outcomes, welcoming challenge and embracing change.

Advanced Social Worker Additional Requirements:

• We are looking for someone who can bring significant 2 years post-qualification experience, including working with individuals with complex needs.
• You must evidence advanced level practice, demonstrating high-quality professional reasoning, sound judgement and leadership in complex situations.
• You will be expected to take a lead role in supporting practice development, contributing to and promoting high standards across the team.
• In line with Advanced Social Worker expectations, you must demonstrate a clear commitment to continuous professional development (CPD).

• To be considered for the Advanced Social Worker role, you must provide clear and detailed evidence within your application. The salary is offered within the following bands, dependent on experience:

- Social Worker: 36,363 - 39,152

- Advanced Social Worker: 40,777 - 44,075

• This role is 30 hours per week. Salary is pro rata to hours worked.
• You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
• Business insurance will need to be added to your insurance policy and you can claim back mileage expenses.
• Please note that due to the nature of this role, it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained.
• This role is hybrid, with a mixture of working at our office in Badminton Road, working from home and working within the community.
