Social Prescriber Link Worker

Health & Wellbeing Northamptonshire Permanent Aug 19, 2026

Salary: £27,000 – £30,000 per annum (pro rata)

Hours: 26 hours per week

Location: Hybrid, Northamptonshire Area

Purpose of the Role

The Social Prescriber (also known as a Link Worker) supports individuals to improve their health and wellbeing by connecting them to non-clinical services and community resources.

Working as part of a multi-disciplinary team, the role focuses on what matters to people, helping them address social, emotional, or practical issues that impact health and wellbeing, including loneliness, housing, financial pressures, long term conditions, and lifestyle challenges. The post takes a person centred, asset-based approach, empowering people to use their own strengths to improve outcomes.

Key Responsibilities
  • Work one to one with individuals referred by GPs and other healthcare professionals.
  • Co produce personalised support plans based on individual goals, priorities, and strengths.
  • Support access to community based activities, groups, and voluntary services.
  • Build and maintain trusted, non judgemental relationships.
  • Monitor progress and provide follow up support where appropriate.
  • Develop and maintain strong links with local voluntary, community, and statutory organisations.
  • Promote awareness of social prescribing across health and community networks.
  • Maintain accurate records and case notes, ensuring confidentiality and data protection.
  • Contribute to service development, supervision, and training.
Person Specification Essential
  • Experience supporting people facing health or social inequalities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage a caseload effectively.
  • Good organisation, problem solving, and prioritisation skills.
  • Understanding of safeguarding procedures.
  • Knowledge of local health, social care, and voluntary sector systems.
  • Confidence in networking and partnership working.
Desirable
  • Experience in social prescribing, care navigation, or similar roles.
  • Background in community development or mental health support.
  • Familiarity with EMIS, Systm One, or similar systems.
  • Relevant qualification in health and social care, community work, or coaching.
Additional Requirements
  • Enhanced DBS check.
  • Flexibility to work occasional evenings or weekends.
  • Ability to travel within and occasionally outside the locality.
  • Additional language skills to support people whose first language is not English (desirable)
