What will the role look like?

As a Wellbeing Coordinator, you’ll play a key role in Linking Leeds. You’ll support individuals to access non-medical support by completing holistic assessments, co-creating personal Wellbeing Plans, and linking people to community groups and services that boost mental, physical, and social wellbeing. The role includes regular networking with local providers and community provisions to facilitate a rich Social Prescribing offer to people accessing our service.

The role requires knowledge of community resources and working across GP practices, community venues, and occasional option to work from home. You will carry a case load of people and assist people to build resilience, independence, and a better quality of life.

We are looking for someone with a passion for reducing health inequalities and promoting diversity, excellent interpersonal and communication skills, with the ability to build trusting relationships. The role requires someone who’s comfortable travelling and working across different locations.

The use of artificial intelligence (AI) generated answers is discouraged. The use of AI may affect the depth and understanding that is required, to demonstrate successful answers to experience and knowledge based questions, in our shortlisting process.
