The Life Coach (ELP) is to provide high quality of daily care to the ELP residents clients. He/She is to supervise and to ensure the well-being of the residents/ clients with intellectual disability and/ or autism. He/She is also to provide training to enhance the development of the residents/ clients in the area of independent living skills, leisure, recreation, sports and career development and participation in the community.

Support residents throughout their ELP journey by conducting screening interviews, coordinating pre-onboarding to post-onboarding processes, and facilitating a smooth transition into independent living.
Empower independent living by providing ongoing guidance, monitoring residents' well-being, conducting regular house visits, and fostering positive and harmonious shared living arrangements.
Build an inclusive community by organising social and recreational activities, strengthening community support networks, and serving as the key contact for resident support, conflict resolution, and emergencies.
Coordinate person-centred care by developing and reviewing Individual Care Plans (ICPs), monitoring residents' health and hygiene, and ensuring compliance with medical, dietary, and care requirements.
Ensure operational excellence by maintaining accurate care documentation, incident records, and a safe, clean, and well-maintained living environment, while supporting the review and enhancement of operational SOPs.
Support quality improvement and reporting through the preparation of KPI reports, participation in Quality of Life (QoL) surveys, and contributing to continuous service improvement initiatives.
Conduct Life Planning sessions and regular reviews to ensure all training goals are aligned with the client's personal aspirations.
Assess individual strengths and needs to tailor training toward resident and caregiver-identified goals. Coach and mentor residents in living skills, leisure, sports, and career/personal development.
Reinforce existing competencies while identifying and scaffolding opportunities to further growth.
Develop and implement training programs in collaboration with internal team members and AHPs.
Ensure the reliable operation of monitoring systems and emergency alert devices as part of the safety framework.
Identify potential hazards and implement proactive strategies to mitigate risks within the living environment.
Participate in Multi-Disciplinary Team (MDT) case reviews to evaluate resident progress and refine support strategies.
Engage regularly with caregivers and social workers to align on family-centred goals and long-term future care planning.
Build and sustain rapport with families, employers, grassroots partners, and volunteers/befrienders.
Mentor residents in navigating public resources, managing personal finances, and ensuring safe travel to work and activities.
Act as a resource bridge, connecting residents to external social support services and community programs.
Min. Diploma in any discipline.
Min 1 year of working experience.
Knowledge and experience in residential, assisted living, home based services, or disability service is an added advantage.
Skills in planning and implementing community activity and training programmes.
Proficient in Computer literacy skills - MS Words, Excel and PowerPoint.
Good interpersonal and communication skills and a good team player.
Able to commit to a 5-day work week on rotating shifts basis (morning shift/ afternoon shift), alternate Saturday/Sunday and public holidays.
