Senior Family Practitioner PO1 - Social Care Non-Qualified

Role purpose:Provide targeted family support to reduce risk, prevent escalation, and improve outcomes for children under Child In Need Planning and Child & Family Assessments

Key outputs:

• Delivers direct, intensive work with children and families, particularly at early help, edge of care, or Child in Need level, to improve outcomes and reduce escalation
• Completes high-quality assessments and family plans, ensuring they are strengths-based, timely, and outcome-focused
• Leads on engaging hard-to-reach families, using relationship-based approaches to improve participation and sustain change
• Provides practical interventions and parenting support, addressing issues such as neglect, behaviour, and family functioning
• Coordinates and contributes to multi-agency plans, working closely with schools, health, police, and early help services
• Acts as the lead professional where appropriate, ensuring effective coordination of support around the family
• Monitors and reviews progress against plans, adapting interventions to ensure sustained change
• Identifies safeguarding concerns early and escalates appropriately where risks increase
• Contributes to assessments and reports, including supporting social workers with information and analysis
• Promotes evidence-based practice, using recognised tools and models for family interventions
• Builds family resilience and independence, reducing reliance on long-term services
• Maintains accurate and timely case recording, ensuring compliance with policies and standards
• Improves outcomes for children, including safety, school attendance, emotional wellbeing, and stability
