Leaders In Care is seeking a Social Worker to coordinate post-discharge planning and ensure safe transitions for patients leaving hospital. You will collaborate with patients, families and healthcare professionals to tailor care plans and secure continuity of care with external providers.
The role offers flexible/Hybrid working arrangements, two days work-from-home, and onboarding support. SWE registration is required, with a £500 referral reward and a DTV for a supportive team.
