Leaders in Care are recruiting for a Social Work role within a Transfer of Care / Hospital Discharge Team. Based in a fast-paced hospital setting, you will support safe discharges and help individuals return home with appropriate packages.
This permanent role offers hybrid working (up to two days remote per week) and a supportive multi-disciplinary environment. You must hold a recognised Social Work qualification and be registered with Social Work England.
