About the Role

Our client is actively seeking a motivated and organized Remote Community Care Coordinator to manage client services and outreach initiatives from anywhere within South Africa. This role is pivotal in connecting individuals with essential support services, developing personalized care plans, and ensuring seamless coordination between clients, service providers, and funding bodies. As a remote position, you will leverage technology to deliver high-quality care coordination, manage caseloads efficiently, and contribute to the overall mission of enhancing community well-being.

The ideal candidate is a self-starter with excellent communication skills, adept at managing responsibilities autonomously.

• Coordinate and manage a caseload of clients, developing and implementing tailored support plans.
• Liaise effectively with healthcare professionals, social services, and community organizations to facilitate client care.
• Conduct remote client assessments via video calls and other digital platforms.
• Maintain accurate and up-to-date client records within the designated CRM system.
• Provide clients with information and guidance on accessing available resources and benefits.
• Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.

• Bachelor's degree in Social Work, Psychology, Public Health, or a related field.
• Proven experience in case management, care coordination, or community services.
• Excellent virtual communication and interpersonal skills.
• Proficiency in using remote collaboration tools and electronic health record systems.
• Strong organizational abilities and time management skills for independent work.
• Ability to navigate and understand various social support systems and resources relevant to the Kwa Zulu-Natal region, though work is remote.
