We’re hiring aSocial Prescriber to join the Live Well Wakefield team, a community-based service that supports adults aged 18+ to improve their health, wellbeing and independence. This role is part of our Healthy Working Life work focused on supporting individuals to remain well at work.

We welcome applications from individuals interested in applying for the role as a new employment, as well as colleagues who may wish to undertake the role on a secondment basis – gaining understanding of the district's largest social prescribing programme and how to create links with your own organisation.

About the role

As a Social Prescriber, you’ll provide information, advice and support for non-medical needs to improve wellbeing and support employment goals. You’ll:

  • Work with people one-to-one to set health and wellbeing goals and address social barriers that may be impacting on their wellbeing and employment
  • Work collaboratively with local VCSE organisations, health and social care, primary care, and local employers as part of multi-disciplinary teams
  • Embed social prescribing within new opportunities and promote the service
What are we looking for in our Social Prescriber?

We’re looking for an individual who:

  • Has experience in supporting people to improve their health, wellbeing and employment, including those with complex needs or from diverse communities
  • Is a great listeners, relationship builders, and natural motivators
  • Is confident working independently and within multi-disciplinary teams
  • Is committed to equality, inclusion, and making support accessible to all
  • Has a strong understanding of employment issues and how these relate to health inequalities
  • Has excellent organisational, communication and IT skills
  • Has a full driving licence and access to a vehicle
