Case Manager - Social Services

About the Role

Our client is looking for a proactive and organized Case Manager to join their social services team, initially serving clients in the Rustenburg area, with the role offering full remote flexibility. You will manage a caseload of clients, assessing their needs, developing personalized care plans, and coordinating access to essential social services. This position requires a deep understanding of available resources and a commitment to advocating for client well-being.

It's a critical role focused on empowering individuals and families to overcome challenges and achieve greater independence through diligent case management.

• Assess client needs and develop comprehensive, individualized case management plans.
• Coordinate services with healthcare providers, government agencies, and community organizations.
• Monitor client progress and adjust service plans as necessary to ensure effectiveness.
• Provide counseling and support to clients facing social, emotional, or economic challenges.
• Maintain detailed and accurate client records, ensuring confidentiality and compliance.
• Advocate on behalf of clients to ensure they receive appropriate services and support.

• Bachelor's degree in Social Work, Psychology, or a related field.
• Previous experience as a Case Manager or in a similar social services role.
• Familiarity with local social service networks and resources in the North West province.
• Strong organizational, time management, and documentation skills.
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients.
• Proficiency in digital communication tools and case management software.
