Position: Social Worker with DAFHT Hybrid Role

Key Responsibilities:

• Assist clients struggling with disability and mental health

• Navigate support systems like ODSP and Ontario Works

• Conduct psycho-social assessments for diverse needs

• Collaborate effectively with interdisciplinary teams

• Manage patient visits in various community settings

Requirements:

• MSW with 2+ years or BSW with 4+ years of experience

• Good standing membership with OCSWSSW

• Case management and system navigation proficiency

• In-depth knowledge of local health services

• Valid driver’s license and reliable transportation
