The Opportunity!
Reporting to the Manager, Developmental Services
- Adult and PDP, the Case Manager, Alternative Level of Care provides intensive, time-limited case management for adults with developmental disabilities who have been designated Alternative Level of Care (ALC), are at risk of inappropriate hospitalization, or require coordinated transition planning to live safely in the community. The role supports person-directed planning, timely discharge, service coordination and access to appropriate community-based supports.

Working closely with clients, families/caregivers, hospitals, developmental services, health partners and community agencies, the Case Manager helps reduce system barriers, strengthen continuity of care and promote dignity, inclusion and quality of life.
Responsibilities:
Provide intensive, time-limited case management to adults with developmental disabilities who are designated ALC, at risk of inappropriate hospitalization, or require coordinated transition planning.
Lead person-directed assessment, planning, implementation and review of individualized service plans that support safe discharge, community inclusion and continuity of care.
Work collaboratively with clients, families/caregivers, substitute decision-makers where applicable, hospitals, developmental service agencies, health providers and community partners.
Coordinate and/or participate in case conferences, discharge planning meetings and cross-sector planning tables to address barriers related to housing, funding, staffing, clinical supports, behavioural supports and community resources.
Support clients and families/caregivers to access appropriate services and supports, including developmental services, mental health supports, behavioural supports, personal support, respite, day programs and residential options.
Provide consultation, education and practical resources to partners regarding developmental disabilities, dual diagnosis, person-directed planning, trauma-informed practice and community supports.
Advocate with and on behalf of clients and families/caregivers for timely access to appropriate services, supports and accommodations.
Identify service gaps, systemic barriers and recurring transition challenges, and share trends with the manager to support program planning, partnership development and system advocacy.
Maintain timely, accurate and confidential documentation, case notes and service data in accordance with funder requirements and FST policies.
Inform the manager promptly of client incidents involving risk, safeguarding concerns or potential serious occurrences.
Participate in team meetings, supervision, case consultation, training, program evaluation, quality improvement and accreditation-related activities.
Travel within Toronto to hospitals, agencies, client homes where applicable, community meetings and planning tables.
Represent FST at community planning, partnership and system coordination tables as requested.
Supervise students and/or volunteers as assigned.
Perform other duties consistent with the role.

Qualifications:

Post-secondary degree in social work, social services, developmental services, psychology, nursing, occupational therapy or a related field, or an equivalent combination of education and experience; foreign credentials and relevant experience will be considered.
