Ferm Werk zoekt een Consulent Re-integratie die inwoners van 27 jaar en ouder begeleidt richting werk, participatie of ontwikkeling. Je voert diepgaande intakes uit, brengt persoonlijke situatie in kaart en stelt een plan van aanpak op.
Je bewaakt voortgang, regie en bijsturing, beheert zelfstandig je caseload, rapporteert zorgvuldig en registreert in Suite en Competensys. Je werkt nauw samen met ketenpartners en levert input voor verbetering van het gemeentelijke re-integratieproces.
