Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week. Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
Job IntroductionWe’re recruiting a Social Work Assistant to join our Assessment, Review and Hospital Discharge Team at Milton Keynes City Council.
Our team plays a vital role in supporting adults at key points in their lives, helping people access the right support, maintain their independence and achieve the outcomes that matter most to them. Working across assessment, review and hospital discharge services, you'll make a direct difference to the wellbeing of local residents and their families.
This rewarding role involves working closely with adults, carers, health professionals and partner organisations to assess needs, review support arrangements and help people transition safely from hospital back into their communities. You'll support individuals to build on their strengths, access community resources and remain as independent as possible.
