About the role

The team plays a vital role in enabling safe, timely discharges from hospital settings by working collaboratively within a multidisciplinary framework. Empowering residents with the tools, confidence, and support needed to transition back into their home environment-whatever form that may take or pathway that may be required. At the heart of the team's work is a commitment to maximising each resident's opportunity to return home and live as independently as possible.

By building on existing support networks and strengthening connections within local communities, the team helps residents develop the skills and resilience to be self-sufficient, while also ensuring appropriate safeguarding measures are in place. Through this coordinated and person-centred approach, the team strives to support residents in living fulfilling, independent lives and achieving the best possible outcomes for their future. We are seeking high calibre and creative applicants who, like us, are committed to building on residents' strengths and their natural support networks, rather than simply assessing people to determine their eligibility for a service.

We are looking for social workers who firmly believe that it is essential to give the residents who we serve as much choice and control as possible over their day to day lives, especially at a time when they have been unwell and still need time to recover and build up their resilience and strength. As a social worker, you will also hold a varied caseload working with residents across the discharge pathways.

You will be completing MCA's,human rights assessments, NRTPF and residents who are homeless, managing complex and challenging/ family dynamics and those residents who require Safeguarding.

We are a Home Office-licensed sponsor; however, for this specific role we are not able to provide Skilled Worker visa sponsorship. To be considered, you must already hold a form of UK work authorisation that does not require sponsorship.
