Job Description
- Engage in meaningful dialogue with patients to gather a comprehensive assessment of their physical, emotional, psychological, and social needs.
- Develop a tailored care plan addressing the client’s specific goals using a client‑centered approach.
- Set measurable goals to provide clarity, track progress, and ensure interventions target specific outcomes.
- Collaborate with a multidisciplinary team to ensure a holistic and coordinated approach to the patient’s care plan.
- Provide ongoing support, offering guidance and encouragement to help patients navigate challenges and achieve their goals.
- Assist with identification, referrals, and navigation of community resources to help patients reach their objectives.
- Conduct regular follow‑up visits (in person or virtual) to assess progress, address emerging issues, and adjust the care plan as required.
- Completion of a Bachelor of Social Work from an accredited university.
- Master of Social Work preferred.
- Registered with the Ontario College of Social Workers & Social Service Workers.
- Minimum five years of experience working as a Social Worker.
- Experience working in a community setting is an asset.
- Valid Ontario driver's license and reliable vehicle.
- Knowledge and experience working with a diverse patient population, including frail seniors and patients with multiple comorbidities.
- Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting).
Salary: CA $28.00 hourly
