About the Role
Our client is seeking a proactive and organized Remote Case Manager to join their community services team, serving individuals and families remotely. This position is crucial for coordinating support and resources for clients, ensuring they receive the assistance needed to achieve their goals. You will utilize digital platforms to connect with clients, assess needs, develop care plans, and monitor progress.
