Social Support Facilitator

About the Role

Our client is seeking a skilled and empathetic Social Support Facilitator to provide essential services on a contract basis. This role is designed for an individual who excels at connecting people with the resources they need to overcome challenges and improve their lives. You will work closely with clients to assess their situations, develop personalized support plans, and facilitate access to social services, counseling, and community programs.

• Conduct comprehensive assessments of client needs, including social, emotional, and practical challenges.
• Develop individualized support plans outlining goals, required services, and action steps for clients.
• Facilitate client access to relevant social services, community resources, government programs, and support groups.
• Provide guidance, encouragement, and emotional support to clients throughout their journey.
• Maintain accurate and confidential client records, documenting progress, interventions, and outcomes.
• Collaborate with other service providers and stakeholders to ensure a coordinated and effective support system for clients.

• A relevant qualification in social work, counseling, social services, or a related field.
• Demonstrated experience in case management, client support, or social services provision.
• Strong knowledge of social welfare systems, community resources, and support networks.
• Excellent assessment, active listening, and empathetic communication skills.
• Ability to work autonomously in a remote setting, manage caseloads efficiently, and maintain high standards of professionalism.
• Proficiency in using digital tools for communication, record-keeping, and client management.
