- Conduct remote client assessments via video conferencing and phone calls.
- Develop, implement, and monitor individualized case management plans.
- Coordinate access to community resources, healthcare, and social services.
- Maintain accurate and confidential electronic client records.
- Provide ongoing support, advocacy, and crisis intervention to clients remotely.
- Liaise effectively with external agencies and service providers via digital communication channels.
- Master's degree in Social Work or equivalent qualification.
- Current registration with SACSSP.
- Minimum of 3 years of experience in case management or direct client service.
- Proficiency in using virtual communication tools (e.g., Zoom, Teams) and electronic record-keeping systems.
- Excellent organizational, time management, and problem-solving skills.
