About the Role
Our client is seeking a skilled and empathetic Social Support Specialist to join their team in the vibrant community of Sandton . This hybrid role offers a dynamic work environment, blending essential on-site client engagement with the flexibility of remote work. You will be dedicated to providing comprehensive social support services to individuals and families, helping them navigate life's challenges and access necessary resources.
This position requires a proactive approach to identifying needs, coordinating interventions, and fostering resilience within the community. Your role is vital in enhancing the social well-being of residents in the Gauteng province.
- Conduct comprehensive social needs assessments and develop tailored support plans for clients.
- Provide counseling, emotional support, and practical assistance to individuals and families in need.
- Facilitate access to community resources, including healthcare, housing, financial assistance, and employment services.
- Maintain accurate, confidential client records and ensure timely documentation of all interactions and progress.
- Collaborate with partner agencies, service providers, and community stakeholders to ensure holistic support.
- Conduct on-site client visits and community outreach, complemented by remote case management and follow-up.
- A relevant degree in Social Work, Psychology, Community Development, or a related discipline.
- Minimum of 4 years of experience in social support, case management, or community services.
- Strong understanding of social welfare systems, community resources, and mental health principles.
- Excellent communication, active listening, and problem-solving skills.
- Proven ability to manage a hybrid workload effectively, balancing on-site and remote tasks.
- Familiarity with the Sandton community and its available resources.
