SAFY's Cleveland division is looking for a Part-Time over 25
Foster Home Worker who is passionate about helping children, supporting foster families, & strengthening communities.

WhatYou’ll Do Clinical:
- Complete case notes to document contacts to all assigned foster homes that do not have foster youth placed in the home.
- (OH) Complete Environmental Audits (F-63) for all Foster Homes without placements.
- (OH) In conjunction with the assigned Case Manager, will complete Foster Parent evaluation annually.
- (OH) Will conduct annual Foster Parent evaluations for Homes without youth.
- Complete annual paperwork for all foster homes without placements and responsible for dissemination and collections of annual documents for homes with placements.
- Complete Home Studies or Amendments as required and collect all paperwork needed for amendments.
- (OH) Amendments are required for all moves into or out of the foster home; when new Household members enter the home, etc.) All Home Study Amendments are approved and signed off by Supervisor.
- (OH) May complete licensing related Rules Investigations of all foster homes assigned by Supervisor.
- May be responsible for tracking Foster Parents training hours, along with tracking all paperwork needed by Foster Parents to remain in compliance with SAFY policies and state specific rules.
- (OH) May complete Large Family Assessment (the state required assessment if a Foster to Adopt home has 5 or more youth).
- Completion of the Individual Training Needs Assessment (ITNA) annually.
- Will conduct foster parent training.
- Update pre-service registration spreadsheet within 24 hours of training. (AL) When FHW completes the training.
- Will attend all Growth & Development Meetings
- Provide trainings to applicants/foster parents on how to use various system and assist with completing/uploading documents into those systems.
- Will offer document assistance sessions in an effort to aid in the licensing process.
- Have full understanding of forms/documents for initial licensing, annual, mid certs and recertification process.
- Timely documentation of home visits, support sessions, interactions with parties involved in the foster home.
- Review all foster home files as part of the quality improvement process.
- Be in compliance with quarterly requirements as defined annually by STARS standards.
- Assist clinical staff with completion of surveys and tools, collect and organize necessary paperwork from foster parents
- (OH) Collect all documentation pertaining to licensing of applicants & Foster Parents.
- While in the home report any concerns/ incidents concerning youth or foster parents immediately to supervisor.
- Map schedule with supervisor and document activities in Outlook calendar. Meet with supervisor a minimum of once a month for Supervision.
- Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.
- Establish and maintain a supportive and positive working relationship with youth, foster parents, referring agencies, and other SAFY stakeholders.
- Attend relevant in-house and external seminars, trainings, conferences and workshops to develop and enhance knowledge in the designated area as well as achieve relevant education hours per year to enhance professional and job-related skills.
- Attends required meetings within division, region, and central office (team and/or community partners).
- Participate in assigned committees and chairs if so appointed. If appointed, maintains liaisons with state and national organizations.
