Remote Case Manager - Social Services

Our client is looking for an experienced and self-motivated Remote Case Manager to join their virtual social services team, serving clients in the Benoni region. In this role, you will manage a caseload of clients remotely, assessing their needs, developing personalized support plans, and connecting them with essential community resources and services. This position requires excellent organizational skills, proficiency in virtual communication tools, and a strong commitment to client advocacy from the comfort of your home office.

• Conduct comprehensive client needs assessments via video calls and phone consultations.
• Develop, implement, and monitor individualized case management plans.

• Maintain detailed and accurate client records within the agency's database system.

• Bachelor's degree in Social Work, Psychology, Sociology, or a related human services field.
• Minimum of 3 years of experience in case management or social work.
• Proficiency with case management software and virtual communication platforms (e.g., Zoom, Teams).
• Strong understanding of social service systems and available resources.
• Excellent active listening, empathy, and problem-solving skills.
• Ability to work independently and manage time effectively in a remote setting, serving clients in the Benoni area.
