A Self-Directed Services Support Broker, under general supervision and review, helps individuals in self-directed service programs design, manage, and control their own supports. Working on behalf of the individual, they focus on promoting choice, independence, and self-advocacy. They assist with person-centered planning, guide individuals in managing their service budgets, and support them in hiring and supervising staff. They assist in developing person-centered plans by helping individuals define their goals and translate them into actionable support, while also providing guidance on managing the individual budget associated with Medicaid-funded services.

This includes helping allocate funds, monitor expenditures, and make adjustments as needed. While they collaborate with case managers, their role is distinctly separate and is centered on coaching, advocacy, and helping individuals navigate systems and make informed decisions, ensuring services are flexible, personalized, and aligned with the individual's goals.

Requirements
  • Possess a high school diploma or certificate of completion of the General Education Development (GED) test; AND have three (3) years of experience in providing supports and/or services to persons with mental illness, developmental disabilities, or substance use disorders.
  • Possess a bachelor's degree in a human service field, such as, psychology, social work, human services, etc. or be eligible to graduate within 90 days of application and able to submit an official transcript upon appointment to position.
  • Possess a valid driver's license or the ability to obtain a Michigan Driver License within 30 days of employment.
  • Must have reliable transportation that may have to be used to carry out job duties of this classification.
Preferred but not Required
  • Some post high-school education is preferred but not required.
  • Bilingual candidates highly desired.
  • Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
  • Individuals in Recovery and individuals with experience in Armed Services valued.

An employee in this class performs generally sedentary work activity requiring occasional lifting of objects weighing thirty-five (35) pounds or less.

Environmental Conditions

An employee in this class generally works remotely or in the offices of Health West, although travel throughout the County by use of personal resources is required to provide face-to-face service to individuals in their home or community setting, facilitate or attend training, and/or for other required job duties.

Evaluation Content

Applicants for County employment may be subject to a post-offer criminal background check, depending on the nature and assignment of the position. When such check is conducted, an offer of employment is contingent upon the results of the criminal background check. Appointees must satisfactorily complete a probationary period before the appointment will be considered permanent.

Purpose

The purpose of this recruitment is to establish an eligible list from which to fill present and future vacancies in this classification in the County system. For each opening, the Department Head has her/his choice of the candidates who meet the required minimum entrance qualifications. This list remains in effect for three months unless exhausted or extended.
