Targeted Case Manager; Bilingual Spanish

This role will require 75% travel for face-to-face visits with members in Tampa area.

From $34.14 per hour

Monday–Friday | 8:00 AM – 5:00 PM No weekends No holidays

We are seeking a Social Work / Human Services Case Manager to support Medicaid Long-Term Care (LTC) and Comprehensive Program enrollees. This is a hybrid role, combining remote documentation with field-based member visits.

Candidates MUST be located in Tampa FL zip codes (33604, 33614 or 33615)

• Coordinate case management services for Medicaid Long-Term Care members
• Conduct comprehensive member assessments using care management tools and data review
• Develop, implement, and monitor individualized care plans
• Collaborate with interdisciplinary care teams to achieve optimal health outcomes
• Track progress and update documentation in accordance with program requirements

Up to 75% field-based travel Travel time paid at hourly wage Mileage reimbursed at IRS rate Reliable transportation required

• Case management experience required
• Bachelor’s degree in Social Work or a related Human Services field
• Candidates MUST be located in Tampa FL zip codes (33604, 33614 or 33615)

• Ability to work independently in the field and remotely
• Authorized to work in the U.S.

• Experience in Long-Term Care (LTC) or Medicaid programs
• Strong organizational, time-management, and multitasking skills
• Excellent verbal and written communication abilities
• Comfortable working in a fast-paced, changing environment

Training 2 weeks of remote training Monday–Friday | 8:00 AM – 5:00 PM Full attendance required
