off the street outreach ohio is seeking a dedicated Outreach Specialist to help connect individuals with community resources and support services.
Position SummaryThe Outreach Specialist plays a key role in expanding community engagement and ensuring individuals can access the support they need. This position involves building relationships, conducting outreach in assigned areas, and coordinating appropriate referrals while maintaining accurate, timely documentation.
Working in a remote capacity, the Outreach Specialist partners with internal team members to support program goals, strengthen stakeholder connections, and provide a consistent, person-centered experience for those seeking services.
Key ResponsibilitiesConduct community outreach to build trust, identify needs, and connect individuals to relevant programs and resources
Engage with clients in a respectful, nonjudgmental manner while promoting participant safety and dignity
Assess basic service needs and make appropriate referrals to internal programs and community partners
Maintain accurate records of outreach activity, referrals, and follow-up outcomes in accordance with program requirements
Support outreach planning and team coordination to ensure coverage of assigned priorities and targets
Demonstrated experience in outreach, customer/client-facing roles, community engagement, or related support work
Strong interpersonal skills with the ability to communicate clearly and professionally with diverse individuals
Ability to work independently and remain organized while completing outreach tasks and documentation remotely
Reliability and strong judgment, with a commitment to confidentiality and ethical conduct
Willingness to support community-centered work that may require flexibility in scheduling
Experience working with individuals facing housing instability, behavioral health needs, or social service barriers
Familiarity with community resources and referral processes, including knowledge of local service systems
Experience documenting outreach and maintaining program records in a structured, detail-oriented way
Ability to collaborate effectively across teams and coordinate with external partners
Hourly rate: $27
