Disability Support Officer; Inclusive

• Provide guidance and practical assistance to clients in areas such as daily living, vocational training, and community participation.
• Connect clients with relevant community services, healthcare providers, and recreational opportunities.
• Educate clients and their families on available resources and adaptive strategies.
• Monitor client progress, document interventions, and report on outcomes.
• Advocate for clients' rights and needs within various settings.
• Facilitate the use of assistive technology and adaptive equipment to enhance independence.
• Conduct risk assessments and develop safety plans in collaboration with clients.
• Organize and support community-based activities and social inclusion initiatives.
• Maintain professional knowledge of best practices in disability support and inclusive living.

• A relevant tertiary qualification (e.g., Social Work, Occupational Therapy, Disability Studies, Psychology).
• At least 4 years of direct experience supporting individuals with disabilities.
• Comprehensive knowledge of disability legislation, support frameworks, and person-centred approaches.
• Strong ability to build rapport and establish trusting relationships with clients from diverse backgrounds.
• Excellent problem-solving, critical thinking, and communication skills (both written and verbal).
• Proficiency in using digital communication tools and case management software for remote work.
• Demonstrated ability to work autonomously and manage a caseload effectively.
• A strong commitment to promoting independence, choice, and inclusion.
• Willingness to travel occasionally for client meetings or community assessments within the Centurion region as required.
• Must possess a reliable internet connection and a dedicated home office space for remote operations.
