Home Care Case Manager

Support elderly clients through home-based case management, service planning, and community social services coordination. This full-time position requires 35 hours weekly and is not remote.
The Case Manager helps elders develop and maintain maximum independent living. Responsibilities include assessing needs, developing service plans and goals, conducting home visits, and maintaining precise records. The position also requires extensive contact with community social services agencies, strong writing skills, and sensitivity to elders’ needs.

Key Responsibilities:

Assess elderly clients’ needs during home visits
Develop service plans and goals for independent living
Maintain precise records and write accurate documentation
Contact community social services agencies extensively
Assist elders in maintaining maximum independent living
Requirements:
Full-time availability for 35 hours weekly
Social work experience and social services knowledge preferred
BA or BS in Human Services, Social Services, or related field preferred
MA Social Work license desirable
Valid driver’s license or reliable transportation and good driving history
