Liberty Community Connections in Chester County, PA, seeks a Service Coordinator to help participants obtain LTSS under CHC. You will lead planning, coordinate services, and facilitate access to medical, social, housing, and educational resources.
The ideal candidate has a bachelor’s degree in social work or a related field, at least three years of relevant experience, a valid driver’s license, and experience with disabilities or seniors needing LTSS. Managed Care experience is a plus.
