Moda Health seeks a Health Coach – Pre‑Diabetes to implement a CDC-approved Diabetes Prevention Program and guide members through lifestyle changes. This is a FT WFH role collaborating with multiple teams and external partners to ensure enrollment and completion of the Pre‑D program.
The position demands strong communication, DEI experience, and knowledge of health behavior change. Flexible scheduling may be required to meet participant needs.
